Phoenix Children’s offers three comprehensive plans from which employees can choose. These plans vary because we know that employees have different needs at different times in their lives. The plans use the United Healthcare Choice Plus Provider Network and are administered through UMR. Note: there is a $25 per pay period additional premium to your health plan if you or your covered spouse use tobacco.

The Employee Assistance Program offers assistance for you and your dependents during time of need.


Phoenix Children's offers three levels of dental coverage from which to choose and include a large network of providers in the MetLife Network.


The vision program is through VSP and providers coverage for routine eye exams, eyeglasses and contact lenses.
