Cystic Fibrosis Family Advisory Council
The Cystic Fibrosis FAC (CF FAC) provides feedback and partners with the CF health care team members. Their goal is to improve patient/family support and care at Phoenix Children's Hospital (pediatrics) and Banner Good Samaritan Hospital (adults).
The council members are committed to the improvement and excellence of the entire Cystic Fibrosis community including pediatric, transitional, post-transplant and adult care.
They aim to create education and support resources for patients and families at the Phoenix Children's CF Care Center.
How can I join?
- If you would like to be considered for membership on the CF Family Advisory Council, please complete an application and submit to firstname.lastname@example.org. The membership is made up of a patient and families living with Cystic Fibrosis. Thank you for your patience in waiting for consideration for membership.
How many members serve on CF FAC?
- 6 - 12 family members serve on CF FAC.
What is the membership commitment?
- Each member serves a two-year term, with an automatic option of a second year of service.
- The council meets the 2nd Wednesday of every month from 10 a.m. – Noon.
Leadership roles rotate within the group on a monthly basis
