Welcome to Phoenix Children’s online Appointment Scheduling! For instructions to schedule an appointment, please refer to the step-by-step directions further down this page.

Note: Please do not use online appointment scheduling for urgent or emergency care. If your child needs immediate care, go to your local urgent care center or emergency department. If your child is experiencing a life-threatening emergency, call 911 immediately.

How to schedule or request an appointment

Available appointments and providers are displayed based on the information you enter regarding the reason for the patient’s visit. Some providers require additional information before you can schedule an appointment. In this case, you will need to request an appointment. You will not be able to request or schedule an appointment with a specialty provider for visit types that are not within their specialty.

Before you get started, please have the following information available:

  • Patient’s name, date of birth and gender
  • Cell phone number for patient or parent/guardian
  • Patient’s home address (for new patients)
  • Email address for parent/guardian (for new patients)
  • Insurance information is helpful, but not required

To get started, complete all requested patient and parent/guardian information above.

On the next screen:

  1. Enter the reason for the visit (choose from drop-down menu if available).
  2. Select the timeframe, day, and time you prefer from the drop-down menus.
  3. Select the program/service for the appointment.
  4. Select the appointment type.
  5. If you have a specific provider, select their name.

To request someone to contact you to schedule an appointment, select Request Appointment.

[Please note, not all programs/services are able to schedule appointments directly via the website. In this case, the only option will be Request Appointment.]

  1. For new patients, add insurance information (if available), parent/guardian cell phone number and email address. Select Submit Request.
  2. For returning patients, review insurance information (if available), parent/guardian cell phone number and email address. Select Submit Request..
  3. You will receive a call to confirm next steps to schedule the requested appointment.

To schedule an appointment, select Find Appointments.

  1. Choose your preferred provider and appointment time, and select Book Appointment to the right of your choice.
  2. For new patients, add insurance information (if available), parent/guardian cell phone number and email address. Select Book Appointment.
  3. For returning patients, review insurance information (if available), parent/guardian cell phone number and email address. Select Book Appointment.
  4. You will receive an email confirming the appointment.
