Patient Alerts

PCCN is responsible for ensuring that patients attributed to its participating primary care practices receive information specific to their health care, care coordination, and important notices regarding gaps in their preventative care. Any message sent under this campaign will be will be sent to the legal guardian for patients in which PCCN is responsible. These messages will support initiatives specific to care coordination and/or addressing a patients gap in care.

Terms and Conditions

Phoenix Children’s Care Network provides patients with various informational messages specific to their enrollment in PCCN's Care Coordination program and ensuring their access to important preventative visits. These messages and alerts are used to provide patients with relevant and timely information related to their care.

Patients may opt-out, opt-in or ask questions about the alerts by calling (602)-933-7226 or emailing PCCN@phoenixchildrens.com. In addition, you can reply HELP for help.

Patients may OPT-OUT of the service at any time by replying to a text message with the word STOP or they may contact PCCN and request that their cell phone number be removed or identified for no messaging.

There is no charge for the service from Phoenix Children’s but message and data rates may apply for SMS text messages received by the patient. Message frequency may vary.

Phoenix Children’s Care Network and mobile phone carriers are not responsible for delayed or undelivered messages.

These messages and alerts are protected under Phoenix Children’s Privacy Policy.
