CARE Program

Some children may find themselves at higher risk for serious conditions like diabetes, obesity or even heart disease. When that happens, it’s important to connect your child and family with the resources and support they need to live a healthier lifestyle.

The Cardiometabolic Risk Assessment, Research and Education (CARE) Program supports children and families to reduce the risk of developing these chronic conditions. And if Type 2 diabetes does occur, the CARE program works with our diabetes program to help manage your child’s condition.

Our team of pediatric specialists will work closely with your child and you to deliver family-centered care. Together, we’ll create a plan that improves your child’s health, boosts their confidence and helps your child feel their best. Services at the CARE clinic include:

  • Resources and support to manage your child’s health condition
  • Medical assessment and monitoring
  • Nutritional counseling with our certified dieticians

The CARE program generally sees patients with a body mass index (BMI) of greater than 85th percentile and who also have one of the following criteria:

  • A1c 5.7% or above (>6.5% please mark as urgent)
  • Fasting glucose 100 or above; or non-fasting glucose >140
  • Fasting triglycerides 130 or above
  • Fasting LDL greater than 130
  • Non-HDL >145

Referrals can be marked as CARE program and faxed to 602-933-2436.

Diabetes Program for Children

Help your child take control of their diabetes with the experience, comprehensive care and support available from the pediatric diabetes program at Phoenix Children’s Hospital. Our team is committed to providing exceptional care that you can trust. We want to help our patients and families feel safe, empowered and confident to manage their diabetes.

We understand that living well with diabetes is much more than just controlling blood sugar numbers. Our team will answer your child’s questions and offer the support and resources to promote your child’s physical, emotional and social health.

The pediatric diabetes program offers a complete range of care for children with type 1 diabetes, and diabetes related to cystic fibrosis, transplants and steroids. We partner with the CARE program for care of those with Type 2 diabetes. Our pediatric diabetes team includes endocrinology physicians, certified diabetes educator nurses and dietitians, and psychologists. We’re committed to advancing the care available for your child and children across – and outside – Phoenix.

Endocrinology Care

When your child’s endocrine system isn’t working properly, it can cause a number of different health conditions and issues. Our pediatric endocrinologists work closely with parents, children, and other specialists to diagnose, treat and manage a wide range of pediatric endocrine disorders.
