Heart Center Family Advisory Council


The Children's Heart Center Family Advisory Council (HCFAC) was formed in June 2013. The council members act as a consultative and advisory resource to Phoenix Children's Hospital Heart Center leadership and staff. The council promotes the core concepts of family centered care: dignity and respect, information sharing, participation, and collaboration.

How can I join?

  • If you would like to be considered for membership on the Heart Center Family Advisory Council, please complete an application and submit to The membership is made up of families who have children with a cardiac illness and have received care at Phoenix Children's Hospital. Thank you for your patience in waiting for consideration for membership.

How many members serve on HCFAC?

  • 8-16 family members serve on HCFAC.

What is the membership commitment? 

  • Each member serves a two-year term
  • The council meets on the 3rd Tuesday of every month from 4:30-6:30 p.m.
