Heart Center Family Advisory Council
The Children's Heart Center Family Advisory Council (HCFAC) was formed in June 2013. The council members act as a consultative and advisory resource to Phoenix Children's Hospital Heart Center leadership and staff. The council promotes the core concepts of family centered care: dignity and respect, information sharing, participation, and collaboration.

How can I join?
- If you would like to be considered for membership on the Heart Center Family Advisory Council, please complete an application and submit to firstname.lastname@example.org. The membership is made up of families who have children with a cardiac illness and have received care at Phoenix Children's Hospital. Thank you for your patience in waiting for consideration for membership.
How many members serve on HCFAC?
- 8-16 family members serve on HCFAC.
What is the membership commitment?
- Each member serves a two-year term
- The council meets on the 3rd Tuesday of every month from 4:30-6:30 p.m.
