Learning from Patient Harm: A Case-based Presentation on Medical Errors and Patient Safety

 

This session will review events in which system errors occurred and resulted in harm to a patient.

At the end of this presentation, participants should be able to:

Objectives:

  1. Review and discuss a patient safety event or known complication that resulted in preventable harm to a patient
  2. Review action plans put in place in response to the serious safety to decrease the likelihood of such events from occurring again
  3. Review current literature regarding types of errors that can occur in our health system and encourage care providers to speak out if they believe there is a potential error occurring
  4. Encourage a culture of transparency and improvement in an ongoing commitment to zero preventable harm
